De niet-simpele ovariumcyste, definitie en diagnostiek
De simpele ovariumcyste is gedefinieerd in richtlijn No 37 van de NVOG, van juli 2001:
“De simpele ovariumcyste is, echoscopisch gemeten, groter dan 3 cm (in drie richtingen gemeten), transsoon, uniloculair zonder septa en zonder papillaire formaties”.
De definitie van de niet-simpele ovariumcyste zou kunnen zijn: de cysteuze afwijking van het ovarium die niet voldoet aan de definitie van de simpele ovariumcyste. Dus groter dan 3 –7 centimeter, multiloculair, al dan niet met septa, al dan niet met aanwezigheid van papillaire formaties, en al dan niet in aanwezigheid van ascites.
De exacte definitie van een niet-simpele cysteuze afwijking in het
kleine bekken is moeilijk. Zelfs met de meest moderne beeldvormende technieken
kunnen aard en oorsprong van tumoren niet altijd worden aangegeven. Ook de
aanwezigheid en het soort klachten van de patiėnt geven geen eenduidige
aanwijzing hierop. Het is een uitdaging voor elke clinicus om een correcte
diagnose te stellen, om vervolgens een optimaal behandelingsplan voor te
stellen.
Complexe cysteuze afwijkingen in het klein bekken zullen meestal van de
adnexa uitgaan, maar soms ook van de organen in de buurt zoals blaas,
urinewegen, dunne en dikke darm. Bij ongeveer 10% van de laparotomieėn verricht
vanwege de verdenking op een ovariumcarcinoom werd een andere tumor dan van het
ovarium aangetroffen (1).
De keuze voor behandeling hangt af van de veronderstelde diagnose en
varieert van eenvoudige observatie (bijv. myoom) tot behandeling met
antibiotica (bijv. PID), met (anti-) hormonen (bijv. endometriosis externa), of
tot minimaal invasieve chirurgie, of uitgebreide chirurgie zoals stagering of
debulking bij ovariumcarcinoom.
De differentiėle diagnose wordt gemaakt met behulp van de
anamnese, leeftijd, menopauzale status, lichamelijk onderzoek (inclusief
recto-vaginaal toucher), beeldvormende technieken (echografie, Doppler flow
imaging, CT-scan, MRI, PET-scan) and laboratoriumonderzoek, zoals serum tumor
markers (bijv. CA 125, CA 15.3, CEA, CA
19-9, α-foetoproteļne).
Wanneer de kans op een maligniteit preoperatief
als hoog wordt ingeschat moet verwijzing naar een oncologisch centrum, cq
operatieve consultatie door een oncologisch gynaecoloog “on-spot” worden
overwogen omdat dit een positief effect heeft op de progressievrije overleving
als op de totale overleving (2-7).
Beeldvormende technieken spelen een cruciale rol
bij de beoordeling van bekkentumoren: echografie, computer tomografie (CT),
kernspintomografie (MRI), positron emission tomografie (PET) of een combinatie
van deze technieken.
Echografie is de belangrijkste diagnostische methode bij de evaluatie van een tumor in het kleine bekken (8-22). De echo kan exact differentiėren tussen cysteuze en solide afwijkingen, en tussen uteriene en ovariėle tumoren. Meestal wordt een combinatie gebruikt van transvaginale en abdominale real-time echografie. De techniek is eenvoudig en de leercurve is kort.
Kleine simpele cysten (< 5 cm in diameter) hebben maar een klein risico op maligniteit (11;18;22-24). Maar een verdikte cystenwand, de aanwezigheid van septa, papillaire formaties of solide delen suggereren een verhoogd risico op maligniteit (25). Er is nog geen overeenstemming over de waarde van de grootte van de cyste, de leeftijd van de patiėnt en de aanwezigheid van een geringe hoeveelheid vrij peritoneaal vocht.
Er
zijn verschillende scoringssystemen ontwikkeld die een betrouwbare en
reproduceerbare uitkomst van de inschatting van de kans op kwaadaardigheid
geven. Allen geven een quasi onafhankelijke semi-kwantitatieve beschrijving van
de tumor (8;9;26-37). Misschien nog belangrijker
dan louter de beschrijving is de garantie van een systematisch onderzoek van
eigenschappen van de tumor die karakteristiek zouden zijn voor de aanwezigheid
van een maligniteit. Een evident nadeel van deze simplificatie is dat evident
benigne, maar niettemin hoogscorende tumoren zoals dermoļdcystes, fibrothecomen
en endometriomata ten onrechte als maligne worden ingeschaald; aan ervaring en
dus subjectieve interpretatie valt derhalve niet geheel te ontkomen (33;38-42).
Nog meer ervaring is nodig wanneer tumoren
worden beoordeeld met Doppler flow imaging. Deze techniek is ter beschikking
gekomen sedert begin van de jaren 90. Angiogenesis (neovascularisatie) van en
naar de tumor is karakteristiek voor
een maligne proces (43). Er ontstaan abnormale flowpatronen, die kunnen
worden gevisualiseerd met behulp van Doppler. De pulsatility index (PI) en
resistence index (RI) kunnen worden berekend. De waarde van Doppler flow
analyse is vergelijkbaar met die van gray-scale imaging (44), lichamelijk (gynaecologisch) onderzoek of
transvaginale echografie (45). Of kleuren Doppler een aanvullende waarde zou
hebben blijft omstreden (46-52) omdat de
specificiteit laag is (13;53-58). Er zijn echter uitstekende resultaten
gepubliceerd door gespecialiseerde centra (32;56;59-71).
Twickler ontwikkelde een Ovarian Tumor Index; hierbij wordt de leeftijd van patiėnte gecombineerd met echografische kenmerken (volume van het ovarium, de Sassone morfologie schaal en Doppler data) (71). De Receiver Operating Characteristic (ROC) curve gaf met deze techniek een area under the curve (AUC) van 0.91, derhalve een hoog discriminerend vermogen aangevend.
Ervaring
is van eminent belang en in tegenstelling tot conventionele gray-scale real-time echografie is het een beperkende
factor voor toepassing in een algemene gynaecologische praktijk. Nieuwe
ontwikkelingen zijn de Power Doppler Angiografie en de 3D echografie. Maar er
zijn nog geen studies waarin een toegevoegde waarde is aangetoond van deze
technieken.
Ook
de analyse van de inhoud van de cyste (kleur, cytologie, meten van concentratie
van oestradiol, progesteron, testosteron, CEA, CA 125 en CA 15-3) kan niet
voldoende differentiėren tussen een benigne en maligne cyste (72). Slechts de combinatie van
negatieve cytologie, lage concentratie van tumor markers, and oestradiol
spiegels groter dan 1000 pg/mL pasten bij de diagnose “functionele” cyste. In benigne tumoren kan actieve secretie of
“shedding” plaatsvinden naar de cyste met als resultaat hoge antigeen spiegels
in de cyste vloeistof, terwijl de serum spiegels laag zijn (73).
Tumor markers
Tumor
markers worden frequent gebruikt bij de evaluatie van processen in het klein
bekken. Sedert de eerste publicatie in 1956 (74) zijn er meer dan 100
potentiėle tumor gerelateerde markers beschreven, o.a. enzymen, hormonen,
niet-specifieke acute fase eiwitten, en placentaire en foetale antigenen. Ze
zijn een reactie op veranderingen van de moleculaire oppervlakte structuur van
het ovarium of een algemene lichamelijke respons op een maligniteit (75). Dankzij de hybridoma
techniek (76) zijn er verschillende
monoklonale antilichamen (MoAb’s) ontwikkeld, die een binding aangaan met
antigenen op de tumor of tumorfragmenten.
Idealiter zou de tumor marker spiegel moeten correleren met de aanwezigheid en
met de hoeveelheid tumor. Een referentiewaarde (cut-off level) die een
onderscheid moet maken tussen normale en pathologische spiegels, moet worden
berekend. De International
Federation of Clinical Chemistry adviseert de
bovenste 95e percentiel in een populatie van gezonde proefpersonen (77).
Deze cut-off levels kunnen wat variėren tussen de verschillende testmethoden en
de verschillende producenten van de tumor markers (78).
CA 125
Het CA 125 antigeen is afkomstig van het coeloom
epitheel (79). Tenminste twee aparte epitope gebieden zijn
geļdentificeerd, die ofwel OC 125-like ofwel M11 antilichamen binden (80;81). In de meeste assays wordt het antilichaam M11
gebruikt als “catcher”, terwijl de OC 125 epitope beschikbaar blijft voor de
kwantificering door de gelabelde OC 125 “tracer”. In de tweede generatie CA 125
assays wordt interferentie met humaan antimuis antilichamen (HAMA ’s) voorkomen
door toevoeging van muisserum. Dit serum neutraliseert deze antilichamen (82).
CA 125 wordt veel gebruikt bij het opsporen en
monitoren van eierstokkanker (83). Hoe hoger het niveau van de marker, des te
hoger de kans dat de tumor kwaadaardig is (84). Als test op zich kan het misschien met enige
zekerheid voorspellen of een optimale debulking haalbaar is (85), maar het levert geen exacte preoperatieve
voorspelling van de soort en aard van het gezwel (86). Eigen studies over de voorspellende waarde van
CA 125 bij tumoren in het kleine bekken gaven een sensitiviteit van ongeveer
70%, een specificiteit van iets meer dan 80%, een positief voorspellende waarde
van 73% en een negatief voorspellende
waarde van 80% (10;17). CA 125 kan verhoogd zijn bij fysiologische
en niet-maligne situaties zoals menstruatie (87-89), zwangerschap (90), endometriose (91-93), lever cirrhose (94), decompensatio cordis (95;96), en bij vocht in het peritoneum, pleura of
pericard, bijv. bij patienten onder behandeling met hemodialyse (97), bij tuberculose (98;99) of andere
infectieziekten (100;101) maar ook
na abdominale chirurgie tengevolge
van het trauma aan het peritoneum. Yedema rapporteerde een sensitiviteit van
84% bij ovariumkanker en van 64% bij kanker, niet afkomstig van het ovarium (102).
Concluderend: CA 125, is naast echografie een
hoeksteen bij de evaluatie van tumoren in het kleine bekken. Maar als test
alleen heeft het onvoldoende discriminerend vermogen. Daarom zijn combinaties
met andere testen essentieel.
De clinicus combineert alle data inclusief zijn eigen subjectieve indruk en maakt een inschatting van de kans op maligniteit. Een exactere en meer wetenschappelijke methode is het gebruik maken van multipele diagnostische testen. Serum CA 125 is in bijna elke studie opgenomen waarbij meerdere testen worden gecombineerd. De testkwaliteiten van de combinaties kunnen worden geanalyseerd door middel van logistische regressie analyse, door het gebruik van de “Risk of Malignancy Index (RMI)”, of door een kunstmatig neuraal netwerk (“artificial neural network”). Logistische regressie analyse is veelvuldig gebruikt bij het analyseren van tumoren in het kleine bekken (10;15;17;34;35;70;103-108). Met behulp van een voorspellend model op basis van logistische regressie analyse blijkt de voorspelling of een tumor benigne of maligne is beter te zijn dan de individuele testen. Een ontwikkeld logistisch model kan ook worden toegepast op een nieuwe studie- of patiėntenpopulatie. Zoals te verwachten is zijn de resultaten vaak minder goed dan in de oorspronkelijke studiepopulatie (108;109). In eigen studies gebruikten we logistische regressie analyse om de bijdrage van de verschillende testen te berekenen aan een statistisch model om de waarschijnlijkheid op een maligniteit te berekenen. Dit model staat op internet (http://pelvicmass.tripod.com) en kan in de dagelijkse praktijk worden gebruikt.
Een andere combinatie van testen werd ontwikkeld door Jacobs et al. (110): de “Risk of Malignancy Index (RMI)”, waarbij gebruik wordt gemaakt van de serum CA 125 spiegel, een echoscore (U = Ultrasound) en de menopauzale status (M) in de formule RMI = U x M x serum CA 125. De toepasbaarheid van deze eerste RMI (RMI 1) werd bevestigd in nieuwe studies van dezelfde onderzoeksgroep (111;112). Door anderen werd de RMI 1 enigszins gemodificeerd naar een RMI 2 (12;15) en vervolgens opnieuw gemodificeerd door deze groep tot een RMI 3 (113). Er zijn geen evidente verschillen in de 3 RMI’s (114). Het voordeel van de RMI’s is zijn simpele toepassingsmogelijkheid zonder dure en ingewikkelde methoden.
Een nieuwe veelbelovende methode zou het kunstmatig neurale netwerk kunnen zijn. Zo’n netwerk is een beslisinstrument, gebaseerd op de structuur en leergedrag van biologische zenuwstelsels die een algoritme ontwikkelt waarin de waarschijnlijkheid van maligniteit berekend wordt. In de radiologie wordt deze techniek al toegepast sinds 1990 (115). Bij bekkentumoren is deze techniek in slechts 3 studies gebruikt: Tailor et al. (116) vonden een sensitiviteit van 100% en een specificiteit van 98%. Timmerman et al. Vonden een sensitiviteit van 95.9% en een specificiteit van 93.5% (117). Zhang et al. (118) vonden een specificiteit van 88%, beter dan de 68% van uitsluitend CA 125 met een vergelijkbare sensitiviteit (79 resp. 82%). Echter de studies zijn klein en een kunstmatig neuraal netwerk is niet altijd superieur aan logistische regressie analyse (119).
Conclusies:
De clinicus moet zoveel mogelijk informatie combineren: anamnese, leeftijd, menopauzale status, lichamelijk (gynaecologisch) onderzoek, met echografie, de eerste keus van beeldvormende technieken. Aanvullende informatie kan worden verkregen met behulp van serum tumor markers, vooral CA 125. Geen enkele methode of laboratoriumtest kan met zekerheid maligniteit aantonen dan wel uitsluiten.
Een
combinatie van verschillende diagnostische testen verbetert de voorspelbaarheid
van de aard van de tumor. Als in zo’n combinatie alle individuele testen niet
suspect zijn voor maligniteit is de tumor vrijwel zeker benigne. Echter het zal
nauwelijks mogelijk zijn om met zekerheid alle maligniteiten preoperatief vast
te stellen, ongeacht de combinaties van verschillende testen of welke
mathematische methode dan ook gebruik wordt.
